PLAYERS PLEASE PRINT THIS PAPER WAIVER FORM, fill it out and give it to your team manager. Note: Players must give this signed waiver form to their team manager prior to taking the field of play for practice or games. Team managers must submit player waiver forms to the League prior to the April start date. New players to the League also need to submit a copy of their photo identification with their waiver form. Opening day is Wednesday, April 18th, 2018.
